Duke Health has announced that it will be offering school sports physicals for a fee of $10 to student athletes in grades seven through twelve. The discounted physicals are set for two dates and locations in May.
Athletes are required to bring with them the North Carolina High School Athletic Association or North Carolina Independent Schools Athletic Association Pre-participation Physical Evaluation Form. The evaluation should have its History Form, pages one and two, signed by a parent or legal guardian. These forms can also be obtained from the athlete’s school athletic trainer or department.
A parent or guardian must accompany the athlete for the physical. However, at the event in Durham, athletes can attend without a parent if they bring a completed permission form. Parking at both venues is free for attendees.
The first session is scheduled for Tuesday, May 20, 2025, at the Duke Physical Therapy Sports Medicine at the Center for Living in Durham. The address is 3475 Erwin Road, and the event will run from 6 to 8 p.m. The second session will take place at Duke Sports Physical Therapy and Occupational Therapy Cary on Wednesday, May 28, 2025. It will be held from 5:30 to 7:30 p.m. at 100 Duke Health Cary Place, Suite 130.
Appointments are required for both events, with limited walk-in slots available. Interested participants are urged to register promptly to secure a spot.
